Abstract
Acute Suppurative Otitis Media (ASOM) is a rapidly developing infection of the middle ear, commonly affecting infants and children. It presents with otalgia, fever, and purulent ear discharge, significantly affecting the quality of life. While conventional medicine offers symptomatic relief through antibiotics and analgesics, recurrence and resistance are common. Homoeopathy, based on the principles of individualization and holistic treatment, offers effective management by addressing both acute symptoms and the patient’s constitution. This article highlights the homoeopathic approach in managing ASOM, supported by materia medica, repertory references, and related literature.

Definition
Acute Suppurative Otitis Media is an acute inflammation of the middle ear cleft with suppuration, usually caused by bacterial infection, often secondary to upper respiratory tract infections. It is characterized by ear pain, fever, and eventually discharge due to tympanic membrane perforation.
Pathophysiology
The infection usually begins as a viral upper respiratory tract infection that leads to Eustachian tube dysfunction. This dysfunction allows bacteria to ascend from the nasopharynx to the middle ear, leading to inflammation, pus formation, and increased pressure. The pressure may cause the tympanic membrane to bulge or rupture, leading to ear discharge.
Etiology
– Infectious causes: Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis
– Predisposing factors:
– Upper respiratory tract infections
– Eustachian tube dysfunction
– Allergies or adenoids
– Bottle feeding in a supine position (infants)
– Environmental factors: smoke exposure, overcrowding
– Immunosuppression
Homoeopathic Approach
Homoeopathy does not treat the disease alone but the diseased individual. In ASOM, treatment is based on presenting symptoms, modalities, constitutional tendencies, and susceptibility. Acute prescriptions help relieve pain and inflammation, while constitutional remedies help prevent recurrence.
Acute Phase Management:
– Select remedies based on totality of acute symptoms: pain type, discharge, laterality, modality, associated symptoms.
Chronic or Recurrent Cases:
– Requires constitutional analysis. Miasmatic background (usually psoro-syphilitic) must be considered.
Homoeopathic Materia Medica
– Belladonna: Sudden onset; intense throbbing ear pain; red, hot ear; high fever; no discharge yet.
– Hepar sulph: Pain worse at night; child is irritable; sensitive to slightest touch; thick yellow discharge.
– Pulsatilla: Thick bland yellow-green discharge; tearful, clingy child; better in open air.
– Chamomilla: Inconsolable child; one cheek red, one pale; oversensitive to pain.
– Merc sol: Profuse, offensive purulent discharge; night sweats; excessive salivation.
– Kali mur: Deafness with snapping sounds in ear; early stage with white discharge.
– Silicea: Recurrent infections; scanty, offensive discharge; low vitality.
– Calcarea carb: Fat, flabby children; profuse sweating; chronic tendency to ear infections.
Repertory References
From Kent’s Repertory and Synthesis Repertory:
– Ear – inflammation – middle ear – acute
– Ear – pain – stitching, tearing, pulsating
– Ear – discharge – purulent, offensive
– Generals – children – affections in
– Face – expression – suffering, crying
Common remedies: Belladonna, Hepar sulph, Pulsatilla, Chamomilla, Merc sol, Silicea, Calcarea carb.
Supportive Measures
– Keep the ear dry; avoid inserting objects.
– Warm compress over the ear to relieve pain.
– Ensure hydration.
– Manage nasal allergies if contributing.
– Educate parents about the correct feeding position in infants.
– Consider adenoid evaluation in recurrent cases.
Brief Review of Related Literature
– A clinical study by Dr. S. Sharma (IJRH, 2019) showed individualized homoeopathic medicines resulted in faster relief in ASOM.
– A CCRH RCT demonstrated effectiveness of Pulsatilla and Hepar sulph in pediatric ASOM.
Conclusion
Homoeopathy provides a safe, effective, and individualized approach to treating Acute Suppurative Otitis Media. Acute remedies manage inflammation and pain effectively, while constitutional treatment prevents recurrences. With proper case taking, repertorization, and remedy selection, ASOM can be managed successfully, especially in pediatric patients.
