Introduction
Haemorrhoids is derived from Greek Word Haima (bleed) & Rhoos (flowing) which means bleeding. It is commonly called as piles which is derived from latin word pila which means ball.

They are divided in to two categories as shown in Figure 1.
- Internal Haemorrhoids means it is within the anal canal & internal to the anal orifice. It lies above the dentate line & is covered by mucous membrane. It is further divided in to Primary Haemorrhoids which lies at the 3,7 & 11 o’clock position & Secondary Haemorrhoids which lies between the primary sites.
- External Haemorrhoids which is situated outside the anal orifice & is covered by skin. It is lies below the dentate line &covered by skin.
- It is caused due to congestion of internal or external or both venous plexus around the anal canal. It is associated with pregnancy, constipation, straining, low fibre diet, obesity, portal hypertension.
Haemorrhoids the classified under four degree.

Degree -1 – Early stage when they bleed but the piles do not prolapse even during defecation.
Degree-2- Piles prolapse during defaecation but it is spontaneously reduced.
Degree -3 – When piles prolapse during defaecation but it needs to be reduced manually.
Degree -4- Piles are permanently prolapsed. Usually infection supervenes &there may be thrombosis of the veins.
Clinical features
Clinical features include bright red painless bleeding seen on the toilet paper or as a fresh splash in the pain. Patient complaints of lumps that appear at the anal orifice.
Homoeopathic Management
1) Aesculus Hippocastanum
Haemorrhoids blind and painful, Rarely bleeding. Severe large piles which seems to block up the rectum. Feeling of dryness in rectum as if little sticks or splinters are there. Haemorrhoids with sharp, shooting pain in the back. Mucous membrane appears swollen and obstruct the passage. Complaints worse during climacteric.
2) Aloes Socotrina
It has marked action on portal congestion. It is best suited to those who has sedentary habits or lifestyle. Haemorrhoids protrude like bunch of grapes which are very sore and tender. Feeling of weakness and loss of power of sphincter ani. Lumpy, watery, jelly like stool.
3) Nitricum Acidum
Haemorrhoids bleed easily. Violent cutting pain in rectum after stool. Splinter like pain in rectum. Haemorrhoids protruding, painless or burning. Rectum feels torn.
4) Collinsonia Canadensis
There is pelvic and portal congestion which results in Haemorrhoids and constipation in females. Sensation of constriction in rectum as if sharp sticks in rectum. Piles are blind or bleeding. Painful and protruding. Obstinate
constipation with Haemorrhoids. Pruritus vulva with Haemorrhoids during pregnancy..
5) Hamamelis Virginica
It acts upon the coats of the veins producing relaxation and consequent engorgement. Haemorrhoids bleed profuselywith sore and raw feeling in anus. Pulsation are felt in rectum. Large quantities of a tar like blood. Itching at the anus.
6) Dioscoreavillosa
Haemorrhoids looks like bunch of grapes or cherries which protrude after stool with pain in anus and darting pain to liver.
7) Ratanhia
Anus aches for several hours after stool as if full of broken glass. Anus feel constricted with sudden knife like stitches. Straining so hard with great protrusion of piles followed for a long time by burning in anus which is temporarily relieved by cold water.
8) Lachesis
Haemorrhoids protrude become constricted and purplish. Tightness in anus with sensation as if nothing would pass through it. Stitching pain in piles on sneezing or coughing. Bleeding from rectum looks like charred straw, black particles. Stool is offensive.
Discussion and Conclusion
Homoeopathy is a system of medicine which is based on the principle of individualization. In this article some important homoeopathic medicines for management of piles are mentioned which are differentiated on the basis of their aetiology, pathogenesis, modalities, character and appearance. Along with homoeopathic medicines, the patient should be advised for maintaining a healthy lifestyle and should be encouraged for a high fibre diet to avoid constipation. Along with this, as a physician we must differentiate among the common differential diagnosis such as anal fissure, anal fistula and rectal prolapse.
