
Abstract– Acne vulgaris is a hormonally influenced inflammatory condition characterized by increased sebum production in hair follicles. This case report describes a 22-year-old female patient with persistent acne vulgaris linked to hormonal fluctuations. The patient was treated with the homeopathic remedy Pulsatilla 200, resulting in significant clinical improvement. Treatment led to a reduction in inflammation, decreased lesion count, and clearer skin without any reported side effects.
Introduction
Acne is a common skin condition that affects millions worldwide. Acne vulgaris is an inflammatory disorder of the pilosebaceous unit that usually resolves on its own but can become chronic. It mainly appears during adolescence in both
males and females and is related to sebum production in hair follicles influenced by sex hormones. Various factors contribute to its development, including genetics, diet, cosmetics, menstrual cycles, and psychological factors. Acne typically manifests as a variety of lesions, starting with comedones.
Grade 1 comedones open / closed
Grade 2 Papule erythema
Grade 3 Pustules.
Grade 4 Many pustules Coalesce to form nodules & crusts.
Acne vulgaris can persist into adulthood and seen more in female Types of Acne:
1. Acne Vulgaris: The most prevalent form of acne, marked by whiteheads, blackheads, papules, and pustules. It commonly appears on the face, neck, chest, and back.
2. Cystic Acne: A more intense form of acne involving painful, deep-seated cysts or nodules that often result in scarring.
3. Comedonal Acne: This type is defined by the presence of comedones— non-inflammatory lesions such as blackheads (open comedones) and whiteheads (closed comedones).
4. Papulopustular Acne: Characterized by red, inflamed papules and pustules, this type can range from moderate to severe in intensity. 5. Acne Conglobata: A rare and severe form of acne involving large, interconnected abscesses and widespread inflammation, often leading to significant scarring.
6. Acne Fulminans: An uncommon and extreme type of acne that presents alongside systemic symptoms like fever and joint pain, typically affecting adolescent boys.
7. Acne Rosacea: A chronic skin condition mainly affecting the face, featuring persistent redness, flushing, and the development of papules and pustules.
8. Acne Mechanica: Triggered by friction or pressure on the skin, this type is often caused by tight clothing, helmets, or sports gear.
Homeopathy provides a holistic and cost-effective approach, with published studies demonstrating its role in the management of acne vulgaris.
Case selection
This case study reports the successful treatment of a 22-year-old female patient diagnosed with moderate acne vulgaris. The patient presented with multiple papules, pustules, and comedones predominantly on the cheeks and forehead, along with associated symptoms of menstrual irregularity, emotional sensitivity, and aversion to fatty foods. After thorough case taking and individualization, the homeopathic medicine Pulsatilla 200C was selected as the constitutional remedy.
Case Report –
Patient Details:
A 22-year-old female presented with moderate acne vulgaris primarily affecting the cheeks and forehead.
Chief Complaints:
∙ Presence of multiple papules, pustules, and comedones on the face ∙ Oily skin with intermittent flare-ups
∙ Menstrual irregularity
Emotional sensitivity and mood fluctuations
∙ Aversion to fatty foods
History:
The patient noted that her acne began in late adolescence and had progressively worsened over the past year. She also experienced irregular menstrual cycles accompanied by mood changes. There was no significant personal or family history of acne.
Clinical Examination:
Physical examination revealed numerous inflammatory lesions, including papules and pustules, as well as both open and closed comedones predominantly over the cheeks and forehead. The skin appeared oily with mild inflammation.
Homeopathic Case Taking:
The patient exhibited emotional sensitivity with tendencies toward tearfulness and a desire for consolation. Aggravation from warmth, heat fatty, food better by cold application, Considering these physical and emotional symptoms, a constitutional remedy was selected.
Repertorial Analysis
Prescription:
Pulsatilla 200C, 4 to 6 globules, administered twice daily.
Treatment Duration:
The patient was treated over a period of three months with monthly follow-up consultations.
Follow-up and Outcome:
∙ After 1 month: Marked reduction in inflammation and fewer new lesions, along with improved emotional stability.
∙ After 2 months: Significant decrease in papules and pustules, reduction in skin oiliness, and improved menstrual regularity.
∙ After 3 months: Almost complete clearance of acne lesions with minimal scarring. The patient reported enhanced overall well-being and satisfaction with the treatment.
Discussion:
Acne vulgaris is a complex condition influenced by hormonal, emotional, and dietary factors. Homeopathy provides a holistic approach, addressing both physical symptoms and the emotional constitution of the patient. Pulsatilla, known for its effectiveness in cases involving skin complaints and emotional sensitivity, was well-suited for this patient. The progressive improvement throughout the treatment period suggests the potential benefit of individualized homeopathic therapy in acne management.
Conclusion:
This case demonstrates the effective treatment of moderate acne vulgaris using constitutional homeopathy with Pulsatilla 200C. Further controlled studies are recommended to better establish the role of homeopathy in acne treatment.
