
Abstract
Atopic dermatitis (AD) is a chronic inflammatory skin condition with a strong immunologic and psychosomatic background. This article presents a detailed homeopathic case study of a 50-year-old female suffering from chronic atopic dermatitis localized to the foot. The individualized homeopathic treatment using remedies such as Petroleum, Gunpowder, and Tellurium showed significant improvement. A holistic approach considering miasmatic background and mental generals played a pivotal role in case resolution.
Introduction
Atopic dermatitis is a common dermatologic condition, typically characterized by chronic relapsing inflammation, itching, and eczematous lesions. Though conventional therapies provide symptomatic relief, homeopathy offers a deeper, constitutional cure by addressing the root cause and the miasmatic disposition of the patient.
Pathophysiology
Atopic dermatitis involves a complex interaction between genetic, immunologic, and environmental factors. Dysregulation in the skin barrier function and immune system leads to increased trans-epidermal water loss, microbial colonization (especially Staphylococcus aureus), and hypersensitivity reactions.
Clinical Presentation
– Chief Complaint: Severe itching, dryness, and fissuring on the soles of feet for the last 3 years.
– Onset: Gradual.
– Location: Bilateral feet, predominantly on the plantar aspect.
– Character: Itching worse at night, deep cracks, bleeding occasionally.
– Associated Symptoms: Burning after scratching, thickening of skin, offensive discharge during flare-ups.
– Aggravation: Winter, bathing, woolen socks.
– Amelioration: Warm application, scratching.
Past History
– History of allergic rhinitis in childhood.
– Recurrent fungal infections of the foot.
– Constipation and acidity off and on.
Family History
– Mother: Rheumatoid arthritis.
– Father: Asthma and diabetes.
– Brother: Psoriasis
Physical Generals
- Thermal Reaction- Hot patient.
- Appetite – Four meals/ day.
- Thirst – Normal, 7-8 glasses of water/ day.
- Desire- Spicy food.
- Aversion – No aversion from anything.
- Urine – Pale in colour.
- Stool – Normal.
- Sleep- Disturbed.
- Dreams- No dreams.
- Prespiration – Only in the face and armpits.
Mental Generals
- Irritable and withdrawn.
- Strong aversion to contradiction.
- Anxiety about health and future.
- Sensitive to remarks and easily hurt.
- Tendency to brood over past insults.
Investigations Done in Atopic Dermatitis
– CBC: Normal.
– IgE levels: Elevated.
– Skin biopsy (previously done): Chronic eczematous dermatitis.
– Patch test: Positive for multiple allergens including dust mites and wool.
Assessment (Totality Of Symptoms)
1. Itching of feet with deep cracks and fissures.
2. Worse in winter and bathing.
3. History of allergies.
4. Offensiveness and discharge.
5. Sensitive and irritable disposition.
6. Brooding nature and aversion to contradiction.
Miasmatic Analysis
– Psora: Itching, sensitivity, and anxiety.
– Sycosis: Thickened skin, tendency for fungal infections.
– Syphilis: Cracks, fissures, bleeding, and destruction.
Predominant miasm: Syphilitic with Sycotic background.
Complications
– Risk of secondary infection.
– Lichenification.
– Sleep disturbance due to severe itching.
– Emotional disturbances (anxiety, depression).

4th Visit | Tellurium 200, one dose | Circular eruptions with offensive discharge; good match for syphilitic miasm; patient presented with ring-like lesions.
Progress And Follow – Up
– After 1st and 2nd visits, notable relief in itching and dryness.
– 3rd visit addressed underlying infectious tendency.
– Tellurium brought about major resolution, especially in discoloration and remaining eruption.
– Emotional state improved; sleep normalized.
Scope of Homeopathy in Atopic Dermatitis
Homeopathy treats the individual holistically. In chronic inflammatory diseases like atopic dermatitis, where emotional and immunologic components are intertwined, it addresses not only local symptoms but also constitutional susceptibilities.
