Abstract:

This case report details the successful homoeopathic management of a 22-year-old female student, Ms. E R, presenting with a persistent dermatological complaint provisionally diagnosed as Tinea Corporis (ringworm). The patient presented with classic symptoms of circular, reddish patches with small eruptions and intense itching and occasional burning sensations in the bilateral axillary and groin regions, lasting for one and a half months. Symptoms were notably aggravated at night and by perspiration and ameliorated by scratching. Following repertorization and material medica consultation, the patient was prescribed Sulphur 0/1 (Fifty millesimal(L/M) potency). Significant and steady amelioration was observed, leading to a complete cure without suppression, highlighting the potential of individualized homoeopathy in managing chronic skin affections.

Introduction:

Tinea Corporis, commonly known as ringworm, is a superficial fungal infection of the skin caused by dermatophytes. It typically presents as annular (ring-shaped) plaques with an elevated, scaly border and central clearing. Predisposing factors often include warm, humid environments, close contact with infected individuals, and poor hygiene, all of which are pertinent to infections in intertriginous areas like the axilla and groin.

Despite prior allopathic treatment, the condition persisted. The homoeopathic approach sought to address the patient’s individualizing symptoms (the ‘Sector Totality’) alongside the general and constitutional presentation to select the simillimum.

Case History:

  • History of Patient Illness: A 22-year-old female student presented with circular, reddish patches featuring small eruptions in the bilateral axillary and groin regions aggravated from perspiration and ameliorated by scratching.
  • The condition had persisted for 1.5 months.
  • Treatment History: Prior allopathic treatment was sought for the skin eruptions, but the condition persisted without resolution.
  • Past History: Nothing significant.
  • Family History: Father has Diabetes Mellitus (indicating a Psoric background).

Physical Generals:

  • Thermal reaction: Ambithermal; however, she shows a distinct preference for open air/fans and feels better during the winter season.
  • Appetite: Good; 2 meals per day.
  • Thirst: Decreased; 1 Liter per day.
  • Desire: Fish2+.
  • Aversion: Cucumber2+, cabbage2+.
  • Urine: 4 times/Day; 2 times/Night; straw coloured.
  • Stool: 1 time/Day; normal in consistency.
  • Sleep: Good; refreshed.
  • Dreams: Unremembered.
  • Perspiration: Increased; aggravates the skin symptoms (itching and burning); odourless.

Mental Generals:

  • The “Patient as a Person” is gentle, likes company and open minded.

General physical examination:

  • Appearance: Moderately-built & nourished, fair complexion. No signs of Pallor, Cyanosis, Clubbing, Icterus, Oedema and Lymphadenopathy.
  • Vitals: BP: 102/70 mmHg, Pulse: 73 bpm, RR: 17 rpm.
  • Local Findings: Localized red circular lesions in intertriginous areas (axilla and groin).

Diagnosis: Tinea Corporis (Ringworm).

Conclusion:

This case demonstrates a clear positive response to the individualized homoeopathic remedy, Sulphur 0/1, based on a complete repertorial and miasmatic evaluation. The patient experienced a steady amelioration leading to a cure of the chronic skin condition without any signs of palliation or suppression. The prognosis for Tinea Corporis is generally good, and this homoeopathic intervention proved effective in resolving the ailment, further supported by essential ancillary measures focusing on hygiene and avoidance of scratching.

Other homoeopathic remedies on Tinea corporis:

Tellurium

This remedy is a significant choice for typical ringworm infections (Tinea circinata or Herpes circinatus). The characteristic features are distinct circular, red lesions. A strong identifying feature is an offensive odor emanating from the affected areas, often occurring alongside eczema located behind the ears and on the back of the head.

Sepia

Sepia is frequently indicated for stubborn, recurring cases of ringworm. The characteristic skin pattern includes ring-like lesions appearing in clusters or as scattered spots. The skin is usually dry and itchy. This remedy often suits patients with a history of suppressed ailments or those exhibiting emotional detachment or indifference.

Rhus Toxicodendron

This remedy is considered when the Tinea eruption includes small blisters (vesicles) on a red base, accompanied by intense itching. Key modalities include relief from hot applications and movement, while symptoms are worsened by cold, damp weather.

Graphites

Graphites is reserved for chronic skin issues where the skin is generally unhealthy and prone to infection. Lesions commonly appear in skin folds (like the groin or behind the ears) and characteristically secrete a thick, sticky, honey-like fluid. The patient is often sensitive to cold, but the itching itself gets worse from the warmth of the bed.

Arsenicum Album

Patients requiring Arsenicum Album experience intense burning in the dry, scaly Tinea lesions. Uniquely, this burning pain is alleviated by heat (a distinct contrast to Sulphur). The patient’s overall state is marked by extreme restlessness and pronounced anxiety.

Psorinum

As a prominent nosode (a remedy prepared from diseased products), Psorinum is particularly useful for highly chronic and frequently relapsing Tinea cases, especially where there’s a history of suppressed skin conditions. The patient is typically very chilly and suffers from intolerable itching which is specifically aggravated by the warmth of the bed. A general feeling of being unwell often precedes the outbreak of the eruption.
