Introduction:
According to the 6th edition  of  Organon of medicine in aphorism number -153:The more striking, singular , uncommon and peculiar (characteristic ) signs and symptoms of the case of disease are chiefly and most solely to be kept in view. For it is more particularly these that very similar ones in the list of symptoms of the selected medicines , must correspond to , in order to constitute it the most suitable for affecting the cure.1
After the identification of the PQRS symptoms of a case , repertory also helped in finding the similumum , by the use of suitable repertory and proper rubrics of the PQRS symptoms of case a similimum can be found. Sinusitis is an infection of the paranasal sinuses that either is bacterial (mainly Streptococcus pneumoniae and Haemophilus influenzae) or is occasionally fungal. It is most commonly associated with an upper respiratory tract infection and can occur with asthma. Symptoms include frontal headache, purulent rhinorrhoea, facial pain with tenderness and fever. It can be confused with a variety of other conditions such as migraine, trigeminal neuralgia and cranial arteritis.2
Case history:
A 21-year Male came with the complaints of headache in the forehead  since 1day  which started getting aggravated on straining the eyes i.e, while doing the agricultural  works whenever the pain was more that time he used to get vertigo which was better on taking rest or by closing the eyes and sitting for a bit.  Along with this he started getting the vertigo from the time of headache , nasal discharge and due to this complaint the patient didn’t get the proper sleep.
ON EXAMINATION –
B.P – 130/70 mm Hg , Right arm supine position
Pulse -71/min , Regular rhythm , moderate volume , vessel wall is palpable
Sinuses – B/L -Frontal sinuses are tender
DIAGNOSIS – ACUTE FRONTAL SINUSITIS
ANALYSIS AND EVALUATION OF THE CASE –

As the symptoms shown by the patient was belong to the acute frontal sinusitis except Along with sinusitis vertigo that was considered  as the uncommon and peculiar symptom in the patient and while repertorizing  through the SYNTHESIS REPERTORY: Rubric which was considered –   Vertigo accompanied by , sinus inflammation of ;  Silicea3, was the only single remedy which was present under this rubric
Prescription –
Rx
1. SILICEA 200 (2packets bedtime )For 2days
Follow up – 1
After 2 days of the medicine the episodes of vertigo was completely absent and the pain was also better. And as the satisfaction to the patient a 1packet of SL (H.S) was given and told him to report on the next day  .
Follow  up – 2
During the next follow up the patient was completely better
CONCLUSION
Sinusitis is an infection of the paranasal sinuses.  This article is focused mainly on treating the case by considering the uncommon peculiar characteristic symptom. This highly worked in this case. On Sinusitis is an infection of the paranasal sinuses repertorization there was  one remedy (Silicea) for this rubric after prescribing the remedy we saw a drastic improvement .
