INTRODUCTIOON: Tinea means fungal infection.1 Tinea cruris  is a pruritic superficial fungal infection of the groin and adjacent skin. Tinea cruris is a dermatophytosis that is commonly caused by trichophyton rubrum or T. mentagrophytes. Tinea refers to superficial infection with one of three fungal genera-Microsporum, Epidermophyton, and Trichophyton. These infections  cause serious chronic morbidity2. Tinea cruris most commonly affects adolescent and young adult males. 3 The infection spreads centrifugally and results in annular patches of varying sizes

CASE PROFILE: A 22 year male in come in our OPD with complaints of Red circular round patchy eruption with intense itching with burning sensation in left thigh which is aggravated evening and night, perspiration. He went for cauterization and external applications with mild relief  but  after sometime, it reappeared as red circular patchy eruption in left thigh with itching sensation.

HOPC: Red circular patchy, large eruption on left thigh for 17 months.  Itching with burning eruption aggravated evening, night and perspiration. Amelioration by open air.

PHYSICAL GENERALS: Loss of appetite, desire for spicy food, salt, sometimes stools hard with constipation with violent rectal pain. Thermal reaction Hot.

MENTALS GENERALS: Fear of darkness, dream of robbers at night, desire for company.

CLINICAL FINDING : Appearance mesomorphic . Height 5’2   weight 54kg  ,

Other generals and systematic examination Finding suggest no abnormal sign.

CLINICAL DIAGNOSIS : Cruris  tinea

TOTALITY OF CASE :

 1.Fear of darkness ++

2.Dream of robbers at night +

3.Desire for Company ,

4. Loss of appetite+++

 5.Sometimes stool Hard+,

6.Desire for spicy food ,salt+,

 7.Red circular , round patchy eruptions on left thigh ,

8.Itching with burning sensation aggravated by Night +

9. Itching with burning sensation aggravated by Perspiration+

10. Itching with burning sensation Amelioration by Open air+

First prescription with justification: After observing reportorial result there are many medicines which may be similimum. Natrum mur 200 one dose was prescribed because it  is highly matching to the totality of the patient. After repertorization on  the basis of totality of symptoms and 200 potency prescribed according to susceptibility of patient and  nature of  medicine & disease. 200CH potency was selected and single  dose  was  given  to  avoid   any unwanted  aggravation. Patient was given placebo for 15 days.

DISCUSSION AND CONCLUSION:

Natrum mur was selected on the basis of totality. In reportorial totality Natrum mur and Sulphur  scored the highest marks which shows the mot similimum with the patient’s complaint. Natrum mur  is covering the highest number of symptoms with marked scoring in reportorial totality and with consultation from materia medica Natrum mur was prescribed. As Natrum mur  is deep acting remedy so only single dose in low potency was prescribed and gave relief to the patient in short duration
